Photo Album Submission Form

1. Please submit a separate form for each pet.
2. Each pet submitted will have an individual page in our photo album with your choice of background and borders.
3. You may submit up to 10 pictures, either by email or mail.
4. Please feel free to write as much or as little about your pet as you would like. You can always send an email to change or add text to your pet's page.
5. Each submission has the option of making up to 3 changes in background and/or color choice.

EMAIL: onlinetfp@gmail.com    Please put TFP Photo Album and both your name and your pet's name in the subject line.

MAIL: The Family Pet, 6005 15th Ave. NW, Seattle, WA, Attn: Photo Album    To assure proper identification and prevent loss, please put your both your name and your pet's name on the back of each picture. Be sure to include a self-addressed, stamped envelope for return.
